Step 2: Fill out the information on Citizen Registration (personal information)

Step 3: In province just select “BACOLOD”

Step 4: For House hold member just click the “ADD Button”

Step 5: Fill up the CAPTCHA CODE FOR VERFICATION

Step 6: PDF file will pop up just click upload and print.

What is the purpose of this, according to the government this is another innovation for comprehensive contract tracing against COVID-19 pandemic by this the carrier of the virus will be detected right away to avoid in contact with another person.
